General Instructions

If you plan on using your practice management software for electronic referrals, follow these instructions. The Referral form is an RTF (rich text file) you can import as a template into your practice management software. Download this file (see the Downloads list below) to your desktop and follow these steps:

Do not open the RTF in Word. This will alter the template fields.

1. Open your practice management software
2. Click the document icon
3. Click “templates”, select import templates
4. Find the Holter Monitoring - CloudHolter referral form and select open. Click File, select save as.
5. Check “available to all users”, and save.
6. Please add the template to favourites if possible, as this will assist your GP’s to find the correct template.

To download the referral template as a .rtf, right‑click the link and choose ‘Save link as…’, then confirm the file ends with .rtf. Do not open it in Word. Import it directly into your practice software. If you are unable to download the rtf file you need as an rtf, you can download the .zip file below and unzip it to access the file you need.
